Athletic Participation Information
Students who plan to participate in sports at Princeton University should review the publication Important Information for Athletes - NCAA Qualified Sports, Club and Intramural Sports.
Every Princeton University student-athlete must be approved for participation by a UHS physician. UHS conducts sports physical exams at the beginning of each academic year. These exams are pre-scheduled.
Students who will participate in intercollegiate sports for the first time at Princeton University must complete and submit the First Year Sports Participation - Athletic Physical Form at the sports physical.
Returning athletes must complete and return the Interim Pre-participation Physical Form to their designated athletic trainer.
Female athletes must also complete and return the Supplemental History Form for the Female Athlete to their designated trainer.
